0 No suicidal ideation or risk documented.
2 Remote history without current symptoms or safety concerns.
5 Current passive suicidal ideation without planning or preparatory behavior.
7 Active suicidal ideation with planning but without intent, recent attempt, or acute means.
9 Active ideation with plan and/or recent attempt within the past 6-12 months.
10 Imminent or extreme risk suggesting immediate emergency intervention.
